Registered Nurse (RN)

Job Duties:

• Demonstrate knowledge of home health and competencies in discipline-specific patient care skills required to provide patient care.
• Perform comprehensive patient assessments, as assigned, and collaborate with a multidisciplinary care team to develop or implement an individualized plan of care as per physician orders that ensure quality to achieve desired outcomes and goals.
• Maintain continuity of patient care by communicating information timely and effectively with all team members providing care to the patient.
• Provide patient episode management per protocol.
• Completes documentation timely, accurately, and at the point of care, according to industry standards and guidelines, including subsequent visits, physician orders, care coordination, etc.
• Educate the patient, caregivers, and family on the aftercare or ongoing care of the patient.
• Perform vital signs and patient assessment during visits.

Qualifications:

• Current license to practice as a Registered Nurse (RN) in Maryland.
• At least one year (+ 1) of experience as a Registered Nurse
• Current CPR certification.
• A reliable method of transportation, current driver’s license, and car insurance coverage
• TB Test or Chest X-Ray.
• Strong interpersonal skills.
• Ability to travel to cases as assigned. (RN)

Licensed Practical Nurse (LPN)

Job Duties:

• Making home visits to clients
• Performing assigned duties, including wound care, treatments, and procedures.
• Following up with executing and adequately documenting doctors’ orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations about client care management and services, utilizing electronic medical records.

Qualifications:

• Licensed Practical Nurse (LPN) in the state of Maryland.
• A minimum of one year of recent, verifiable experience as an LPN.
• Current CPR certification.
• A reliable method of transportation, current driver’s license, and car insurance coverage
• TB Test or Chest X-Ray
• Strong interpersonal skills.
• Ability to travel to cases as assigned.

Certified Medication Technician

Job Duties:

• Provides for patient’s personal hygiene and daily living.
• Works effectively as a team member in the delivery of care.
• Assist patients with medications, treatments, and health monitoring – and respond to their needs compassionately.
• Excellent communication skills to report changes and updates on patient conditions.

Qualifications:

• High school diploma or equivalent.
• Certified Medication Technician (CMT) certification is required
• Current CPR certification.
• A reliable method of transportation, current driver’s license, and car insurance coverage.

Home Health Aide

Job Duties:

• Maintains a clean, safe, and healthy environment.
• Assists patients with getting in and out of bed, bathing, hygiene, and toileting.
• Prepares and provides and/or assists with meals.
• Performs light homemaking and other environmental services.
• Observes, reports and documents any changes in patient status.
• Understands essential elements of body functioning and reports changes in patient’s body functions as indicated.
• Recognizes emergencies and implements appropriate emergency procedures per policy.
• Follows procedures regarding infection prevention and control, handling of hazardous waste, and safety measures.
• Follow specific written instructions for personal care under the plan of care.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs other duties as assigned.

Qualifications:

• Valid Maryland Board of Nursing Certified Nursing Assistant (CNA) – not required.
• Current CPR certification.
• Comfortability using technical tools and software for visit documentation.
• A reliable method of transportation, current driver’s license, and car insurance coverage
• TB Test or Chest X-Ray
